Forms: Bring the completed forms to your first session for us to review together. Give yourself about 30 minutes to complete the forms.

Everyone: Download and read the Welcome, Expectations & Confidentiality Form You do not have to print these out, unless you would like a hard copy.

Adults: Print and complete the Adult Intake Packet. For couples, please print two copies to be completed separately.

Parents: Print and complete the Child Intake Packet.

Average cost per hour is $120 - $150.

I accept cash, personal checks, and credit cards. I can also run Health Savings Account (HSA) cards. There is a $4 card-processing fee.

While I do not bill insurance, I can provide a receipt for an out-of-network reimbursement.

Coming from I-24 East (from I-75):

· Continue I-24 West onto US-27N for Chattanooga

· Take Exit 1 (Carter Street) from US-27 N

· Turn right onto Carter Street and go approximately 500 feet.

· 1401 Carter Street Office Building is located on the right

Coming from I-24 West (West of Chattanooga):

· Continue I-24 East onto US -27N for Chattanooga

· Take Exit 1 (Carter Street) from US-27 N

· Turn right onto Carter Street and go approximately 500 feet.

· 1401 Carter Street Office Building is located on the right

Coming from US-27N (heading south to Chattanooga):

· Continue US-27S for Chattanooga

· Take Exit 1A - ML King Blvd in Chattanooga

· At the ML King Blvd traffic light, turn right onto Carter St., approximately ½ mile (just past the Chattanooga convention center).

· 1401 Carter Street Office Building is located on the right.
